Somebody is a little weaker than they were, and their vision has changed slightly, and there is a new medication, and there is a rug in the hallway. Any one of those is survivable. Together they are why someone ends up on the floor. So we test all of it, treat what is trainable, adapt what isn't, and go and look at the house.

What the First Hour Involves
Nothing strenuous, nothing frightening, and you will leave knowing your actual numbers.
The History, Properly Taken
Falls and near-misses, what you've stopped doing, your medications, your eyesight, and the layout of your home. The near-misses matter as much as the falls and are almost never volunteered.
Standardised Balance Testing
Berg Balance Scale, Timed Up and Go, gait speed and a sensory-organisation test that tells us which of the three balance systems is letting you down. All of it seated or with support close by.
Strength and Vestibular Screening
Sit-to-stand testing, grip, ankle strength, and positional testing for vertigo. If it turns out to be BPPV we will often treat it in that same appointment.
Your Numbers, Explained
What each score means, how it compares to your age group, and which one we'd move first. Written down for you and for your family, because most people don't remember it all afterwards.
The Home Visit
Within the first fortnight, at no extra cost. We walk through your house with you, list what to change, and — where it is a rug or a lightbulb — often just do it while we're there.

We Ask About Near-Misses
Almost nobody reports the grab at the counter or the stumble on the step, because nothing happened. Those are the best early warning we have, and asking directly is most of why we catch things a year earlier than we otherwise would.
We Load You Properly
The commonest failure in geriatric rehab is treating older adults as fragile. Muscle adapts to load at any age. Under-dosing is not caution — it is a slower route to the same fall.
We Go to the House
Half of falls happen at home, and no amount of clinic balance work fixes an unlit stair. It is included with every plan because a plan that skips it is only half a plan.
We Talk to the Family
With your permission, and usually gladly. Adult children are often the ones who noticed something first and the ones who'll help make the changes stick.

Our advice, for what it's worth: don't lead with falling. Lead with the thing they've stopped doing — the market, the church steps, the walk to the mailbox. People will engage with getting something back far more readily than with a risk they'd rather not think about.

What People Ask Before Booking
Am I too old for this to work?
No. Balance and strength both improve at every age we have data for, including into the nineties. Our oldest patient last year was 96 and improved her Berg score by eleven points. The rate is slower than at forty; the direction is the same.
Does Medicare cover it?
Yes. Physical therapy for balance and falls prevention is covered under Medicare Part B, and Florida's direct access law means you don't need a physician referral to be evaluated. We verify your benefits before your first visit and tell you your per-visit cost.
I haven't actually fallen. Is it too early?
It is exactly the right time. The best outcomes we get are with people who noticed they were holding the counter more, or had stopped walking on uneven ground. Waiting for a fall means starting from further back.
What is the home visit — is it a sales call?
It is a therapist walking round your house with a checklist, and it's included. Lighting, rugs, thresholds, bathroom rails, the garage step, where the phone is. We don't sell equipment and we have no arrangement with anyone who does.
My dizziness comes and goes. Can you help?
Very likely. The commonest cause of positional vertigo in older adults is BPPV, which is diagnosed with a simple positioning test and usually resolved with a manoeuvre in one or two appointments. It is frequently attributed to blood pressure or age and left untreated for years.
Can you come to me?
For most people the clinic is better — the equipment is here and the testing is more precise. But we do home-based care for people who genuinely cannot travel, and the home safety visit comes to you regardless.
