Rebuild It Right: Orthopedic Physical Therapy in Pittsburgh, PA
Board-certified orthopedic specialists who follow your surgeon's protocol to the week, test your progress instead of guessing at it, and hand you the numbers every step of the way.
Orthopedic Rehab, Built Around the Repair
Every one of these starts with a full evaluation and ends with objective testing — not with a therapist deciding you look about right.

Sixteen Years of Surgeons Sending Us Their Patients
Ironvale opened on East Carson Street in 2010, started by two therapists who had spent a decade in hospital outpatient departments watching good surgical work get undone by rushed rehab.
The model has not changed since. One therapist, one patient, the full hour — not a therapist split across three tables while aides run the exercises. Your surgeon's protocol is read before your first visit and followed to the week. Progress is measured with force plates, dynamometry and hop testing, and the numbers go on a sheet you keep.
That is why the referrals come from surgeons rather than from advertising. Three of our therapists hold the OCS — the board certification in orthopedic physical therapy, which fewer than one in ten American PTs carry.
- ✓One therapist per patient, the full hour
- ✓3 of our therapists hold the OCS board certification
- ✓Your surgeon's protocol read before your first visit
- ✓Objective testing every four weeks, numbers on paper
Four Things Most Clinics Don't Do
None of these are exotic. They are simply the difference between rehab that is measured and rehab that is hoped for.
We Read the Operative Report
Graft choice, fixation, meniscal repair, anchors — these change the timeline substantially. A protocol written for a patellar tendon graft is wrong for a hamstring one, and following the wrong one costs you months.
We Test Instead of Guessing
Force-plate strength testing, handheld dynamometry and hop batteries at four-week intervals. Returning to sport under 90% limb symmetry roughly quadruples your reinjury risk, and eyeballing it cannot find that.
We Load You Properly
The commonest failure in orthopedic rehab is under-dosing. Tissue adapts to load, and a resistance band at week ten is not load. We progress to real weight as soon as the repair can take it, and not a session earlier.
We Discharge You
A plan of care has an end. When you hit your criteria we test you, write it up, send it to your surgeon and let you go — with a maintenance programme, not a standing appointment.
Where the Work Happens
Six thousand square feet on East Carson, built for loading rather than for looking calm.

You Do Not Need a Referral to Start
Pennsylvania has direct access, which means you can be evaluated and treated by a physical therapist for up to 30 days without seeing a physician first. Most people do not know this, and it costs them weeks.
We are in-network with Highmark, UPMC Health Plan, Aetna, Cigna, UnitedHealthcare and Medicare. Before your first visit we verify your benefits and tell you your per-visit responsibility, your deductible position and how many visits your plan authorises. If you are paying cash, the rate is published and there is no difference in the care.
If your case does need a physician — and some do — we will tell you in the first session and help you get there rather than treating you for six weeks first.
- ✓30 days of direct access in Pennsylvania — no referral needed
- ✓In-network with Highmark, UPMC, Aetna, Cigna, UHC, Medicare
- ✓Benefits verified and quoted before your first visit
- ✓Published cash rates, identical standard of care
Sixteen Years of Getting People Back
My surgeon sent over a twelve-week protocol and Ironvale actually followed it — week by week, with the milestones written down where I could see them. I've been through rehab twice before at places that handed me a photocopied sheet. This was the first time I knew what week I was in and why.
Total knee at 68 and I was convinced I'd never get back on the incline trails. They tested me rather than guessed — strength, range, single-leg balance — and showed me the numbers every four weeks. Fourteen weeks later I walked the Grandview steps without stopping.
I came in to avoid shoulder surgery, not to recover from one. They were honest that it might not work and told me exactly what would tell us either way by week six. It worked. I still have my own rotator cuff.
The insurance side was the part I dreaded and it was the easiest bit. They ran my benefits before my first visit and told me the per-visit number over the phone. No surprise bill three months later, which I cannot say about the last clinic.
Most Evaluations Are Booked Within 48 Hours
Bring your operative report if you have one. If you don't, bring the injury and we'll work from there.
What Patients Ask Before Booking
Do I need a referral from my doctor?
No. Pennsylvania's direct access law lets a licensed physical therapist evaluate and treat you for up to 30 days without a physician referral. Some insurance plans still require one for reimbursement, which is exactly the sort of thing we check for you before your first visit.
How soon after surgery should I start?
For most orthopedic procedures, within a few days — often before the sutures come out. Early controlled motion is protective, not risky, and the protocols we follow reflect that. We will coordinate the start date with your surgeon's office directly.
Will I see the same therapist every visit?
Yes. One therapist owns your case from evaluation to discharge, and you get their full hour rather than fifteen minutes between two other tables. Continuity is most of why our outcomes hold up.
How many visits will I need?
Your written plan gives an expected range at the first visit — typically 8 to 12 for a straightforward orthopedic case, 24 to 36 across the full arc of an ACL reconstruction. You will know the number before you commit, and we re-forecast it at every re-test.
What is blood flow restriction training?
A calibrated cuff briefly restricts venous return from a limb, which lets light loads produce strength and muscle gains normally requiring heavy ones. It is why post-operative quadriceps come back weeks earlier than they used to. Our therapists are certified in it and screen you for suitability first.
What does it cost if I don't have insurance?
Our cash rates are published: $165 for the initial evaluation and $110 per follow-up, with a discount for pre-paid blocks. The care is identical — the only difference is the paperwork.
