Nineteen Years of Toledo's Injured Workers

Havenridge opened on Monroe Street in 2007 to do occupational rehabilitation properly, in a city where a great many people do physical work for a living.

We have been inside most of the plants, warehouses and job sites in Lucas County, and that matters more than it sounds. When someone tells us they run a press, unload trailers or turn patients, we know what the job asks of a body — so the rehab targets it and the return-to-work recommendation is realistic.

We are a BWC-certified provider and we work with every major MCO in Ohio. The claim paperwork is done here, by staff who do it every day, because an injured worker chasing an authorisation is a worker who isn't getting better.

You Choose Your Provider

In Ohio you have the right to pick your own BWC-certified physical therapy provider. You do not have to go where you are sent, and you can change providers if the current one isn't working out.

It Costs You Nothing

With an allowed claim, treatment is billed to the BWC or your employer's MCO. No co-pay, no deductible, no bill to you. We verify the claim before your first appointment so there are no surprises.

Light Duty Is Not a Trick

Returning to restricted duty usually produces better outcomes than sitting at home, provided the restrictions are real and enforced. We write them precisely so nobody has to interpret them.

Say If It Still Hurts

Under-reporting to seem cooperative is common and it backfires — it produces a release you cannot sustain and, often, a second injury. Tell us the truth and we'll document it.

Built to Look Like Work

Eight thousand square feet on Monroe Street, and about half of it is a work-simulation floor rather than a clinic.

There is a pallet rack, a conveyor section, a ladder, a crawl space, a valve wheel and a bench at four different heights. Rehab that doesn't resemble your job doesn't transfer to it — a person can be pain-free on a treatment table and still unable to sustain a twelve-hour shift, and that gap is where second injuries come from.

So the last phase of every programme happens out there, doing the actual movements at the actual loads, until the numbers say you're ready.

48-Hour FCE Reports

Standardised, validity-tested and written so they can be acted on. Two working days from the evaluation, not two weeks — because a claim sitting open while a report is drafted costs real money.

Post-Offer Employment Testing

Job-specific physical testing after a conditional offer, built from an actual job analysis and applied consistently to every candidate for the role. Legally defensible and it measurably reduces first-year injury rates.

On-Site Job Analysis

We come to your floor, measure the task and film it. The output is a quantified physical demands description that every subsequent evaluation is measured against — and often a fix that prevents the next injury.

Weights, heights, frequencies and durations, in plain language a supervisor can roster around. No vague 'light duty' that leaves everyone guessing and someone re-injured.

What People Ask Us

Can I choose to come here?

Yes. In Ohio you have the right to select your own BWC-certified provider, and you can change providers during a claim. If you've been sent somewhere that isn't working for you, you are allowed to move.

What if my claim hasn't been approved yet?

Come in anyway. We'll evaluate you, start what we safely can, and help you get the filing right. Delays waiting on paperwork cost recovery time you don't get back, and claims are frequently approved retroactively.

Will this cost me anything?

With an allowed claim, no — treatment is billed to the BWC or the employer's MCO, with no co-pay or deductible. We verify coverage before the first appointment and tell you straight away if there's a problem.

What actually happens in an FCE?

About four hours of standardised testing: lifting from different heights, carrying, pushing and pulling, positional tolerance, grip and repetitive tasks. Validity measures are built in. You'll get breaks, and it is designed to find your safe maximum, not to catch you out.

My employer wants me back on Monday. Can you help?

We can give you objective, defensible numbers on what you can safely do now — and a staged plan to full duty. Return-to-work dates argued from measurement rather than opinion are better for everyone, including your employer.

Do you work Saturdays?

Saturday mornings, seven to twelve, and weekdays from six. Between them most shift patterns are covered without losing hours.
