The short answer
Most US practices charge $10–$20 a unit for Botox, and premium urban markets — New York, Los Angeles, Miami — $18–$35; a typical session runs $300–$800 in standard markets (IAPAM, April 2026). Zenoti, a med spa software company, puts current rates at $10–$18 a unit and its tiers at $9–$12 for volume clinics, $13–$16 mid-market and $17–$25+ premium (July 2026).
By the area, CareCredit's figures are $180–$375 for frown lines (12–25 units), $60–$240 for crow's feet (4–16 units) and $60–$300 for forehead lines (4–20+ units), with a $445 national average treatment (July 2026); the American Society of Plastic Surgeons' latest average for botulinum toxin injections is $435.
What a unit costs you: a 100-unit vial runs about $400–$600 before rebates (IAPAM), $4–$6 a unit. Loaded with waste, consumables, the injector's half hour and overhead, Zenoti's 40-unit treatment costs about $327–$382 — so at $14 a unit ($560) the gross margin is 32%–42%, against 64% on product alone in Zenoti's other example ($5 a unit).
Filler runs $550–$650 a syringe in a low-cost market, $700–$850 mid-market and $900–$1,200+ in a premium one (Zenoti, North American market data). Injectable memberships in Zenoti's example run $99–$299 a month, most with a monthly unit credit. In the US, who may inject is set state by state; check your state's rules before you list a treatment.

What the market, your positioning and the calendar do to the price
A premium city market lifts the per-unit price by about 75%–80%; a volume clinic prices about a quarter below mid-market; some practices add a premium in peak periods.

THE PRICING FORMULA
How to price injectables
Six steps from the cost of a unit to the price list on your website.
Know your cost per unit
A 100-unit Botox vial costs about $400–$600 before rebates (IAPAM, April 2026), $4–$6 a unit. Use your average cost per unit, not your latest invoice, and count the waste — Zenoti's example allows about 12%.
Load in the injector's time and the room
Zenoti's 40-unit example: $240 of product, $29 of waste, $8 of consumables, $35–$75 of provider time and $15–$30 of overhead — about $327–$382 before any profit (July 2026).
Price from a margin
Price = cost ÷ (1 − margin), with a 60%–70% gross margin the common target (Zenoti). Check which cost you mean: $14 a unit on $5 of product is 64% in one Zenoti example; on its loaded 40-unit cost, built on $6 a unit, it's 32%–42%.
Check it against your market
Standard US markets $10–$20 a unit, premium urban $18–$35 (IAPAM). AmSpa's guideline, as Zenoti reports it: stay within 20% of the competitive norm unless you have a clear reason to be higher.
Charge by the unit, sell by the area
Zenoti's recommended mix: per-unit by default, per-area for new patients who want one number ($249–$350 an area — $350 glabella, $299 crow's feet in its example), and a flat full-face session for the top tier ($799 in its example).
Bank the units
A unit bank sells 100 units at $12.50 against a $14 list, good for 12 months; injectable memberships run $99–$299 a month, most with a monthly unit credit — 100 members at $149 is $178,800 a year (Zenoti).
The formula, worked through
Take Zenoti's 40-unit treatment: about $327–$382 once product, waste, consumables, the injector's half hour and overhead are counted.
At $14 a unit the treatment is $560 — a 32%–42% gross margin once everything is loaded, even though $14 against $5 of product alone (Zenoti's other example) looks like 64%. At $16 a unit, the top of Zenoti's mid-market tier, it's $640 and 40%–49%. Zenoti calls 32%–42% a healthy margin; the point is to price against that loaded cost, not against the vial.
So “Botox $14 a unit, frown lines from $250, filler from $700 a syringe, membership from $149 a month with 30 units banked” is a list that sits inside the published mid-market bands. The arithmetic is ours; the figures are the publishers'.
What to charge in your part of the country, and the rules
Market sets the per-unit price more than anything: IAPAM puts standard US markets at $10–$20 a unit and premium urban markets — New York, Los Angeles, Miami — at $18–$35, with sessions of $800–$1,200+ there against $300–$800 elsewhere (April 2026); CareCredit's per-unit average spans $11–$35. Across all prices, the Bureau of Economic Analysis puts California at 110.7 against a US average of 100 and Arkansas at 86.9 (2024 regional price parities, released February 2026).
Who may inject is a state rule. In the US, the American Med Spa Association's guidance is that neurotoxin injections “must be performed by a licensed physician, independent practice nurse practitioner or another licensed medical professional,” that a trained medical assistant may inject only “when ordered and supervised by a physician” where the state allows it, and that “these rules vary by state” (AmSpa, June 2021). Check your state's medical and nursing boards — AmSpa's advice is an experienced health care attorney — before you put a treatment on your price list. This page gives prices only; it isn't medical or legal advice.
Put your prices on your website
A price list — even “from” prices — answers a customer's first question before they pick up the phone, and it's the line an AI assistant can quote when someone asks what a med spa treatment costs near them.
If you build your site with Zarla, tell the editor “add a pricing section: Botox $14 a unit, frown lines from $250, dermal filler from $700 a syringe, memberships from $149 a month” and your prices go on the site, in a section you can edit any time. Med spa pricing: common questions
$10–$20 a unit in most US markets and $18–$35 in premium urban ones (IAPAM, April 2026); Zenoti's tiers are $9–$12 for volume clinics, $13–$16 mid-market and $17–$25+ premium (July 2026). CareCredit's average is $11–$35 a unit. The calculator above uses both.
Both, for different patients: Zenoti recommends per-unit by default, per-area for new patients who want one number ($249–$350 an area in its example), and flat-rate sessions for a top membership tier. CareCredit's per-area figures: frown lines $180–$375, crow's feet $60–$240, forehead lines $60–$300.
About $400–$600 for a 100-unit vial before rebates, $4–$6 a unit (IAPAM). Zenoti's fully loaded 40-unit example — product, waste, consumables, the injector's time and overhead — comes to about $327–$382.
$550–$650 a syringe in a low-cost market, $700–$850 mid-market and $900–$1,200+ in a premium one (Zenoti, July 2026); its FAQ puts mid-market at $650–$950 and premium physician-led practices at $1,000–$1,400.
Zenoti's example tiers: $99 a month for a 20-unit monthly credit, $149 for 30 units plus 10% off more, $199 for 20 units and a $50 filler credit, $299 for full quarterly Botox and a filler syringe every six months; a unit bank of 100 units at $12.50 against a $14 list, valid 12 months (July 2026). IAPAM puts the effective per-unit price under a membership at about $10–$14 in standard markets.
In the US it depends on the state. AmSpa's guidance is that injections must be performed by a licensed physician, independent practice nurse practitioner or another licensed medical professional, that a trained medical assistant may inject only when ordered and supervised by a physician where the state allows it, and that the rules vary by state (June 2021). Check your state's boards and a health care attorney before you list the treatment.
Yes, for the treatments people shop on price — a per-unit Botox price and a “from” price for filler answer the first question before a consultation. Zenoti advises practices not to print prices for body contouring. On a Zarla site, tell the editor “add a pricing section” with your menu, and change it any time.
